Admission registration for direct patients

Patients referred by outside doctors or directly seeking admission can get their registration done at the first floor reception counter for general wards/ second class or first class rooms. —and– at the office on the second floor for admission in iccu/super deluxe/deluxe/ semi deluxe or sharing rooms.

Once registration is done, then the patient gets a permanent id number, which can be referred to in future if further hospitalization required. A written consent is mandatorily documented for all routine medical and surgical patients.

Appointments

All patients need to be registered prior to consultation.
If you are already registered, kindly mention your Paramount Hospital UHID number when taking an appointment.

The registration fee (one time charge) is Rs. 100/. You will be issued a UHID card which is valid for all services of the hospital. Your medical record folder will be retained with the hospital.
In case you are a non-appointment/walk-in patient, you may have to wait to see the doctor.

You can visit our OPD from Monday - Saturday from 9.00am to 8.30pm.
